Community Pharmacist-led Anticoagulation Management Service-CPAMS

The Pharmacy Association of Nova Scotia, in partnership with Doctors Nova Scotia and the Nova Scotia Department of Health and Wellness will be launching the Community Pharmacy Anti-Coagulation Management Demonstration Project (CPAMS) in January 2018.  

 

Forty pharmacies throughout Nova Scotia will be selected to offer the service, which will include publicly funded point of care INR Testing and warfarin management services, at no charge to the patient.  Pharmacists at these locations will be trained and accredited by the University of Waterloo’s Management of Oral Anticoagulation Therapy Program (MOAT), to provide this service.  Patients will be referred to local pharmacies by their family physician.  A collaborative management plan between physician and pharmacist will be in place for each patient.  This plan will clearly outline the target INR, testing interval and a management plan for INRs which fall outside of a pre-determined range.
