NS Seniors Pharmacare (age 65+) who take 4 or more prescription medications for chronic conditions and also meet one of the following criteria, may be eligible for a medication review. If you feel you are experience a drug related problem, if you are taking 8 or more medications and don’t have family physician or NP following your care, or you are referred by a physician/NP or other eligible provider.

A pharmacist will meet one-on-one with you to discuss your medical conditions, medications, lab tests and questions or concerns you have. At the end, you will receive a detailed medication list with the purpose of each drug indicated. Pharmacist will work with you and your healthcare provider to find solutions to any medication concerns. Patients are expected to bring all of the medications that they use to their appointment.  This included over the counter medications, prescription medications, puffers, creams, natural health products.
