Pharmacare patient, who is taking 4 or more prescription medications that are used for the treatment of at least 1 chronic condition, is eligible for a medication review if they are having difficultly with adhering to an understanding their medication regimen OR if they have been recently discharged from hospital.

A pharmacist will meet one-on-one with you to discuss your medical conditions, medications, lab tests and questions or concerns you have. At the end, you will receive a detailed medication list with the purpose of each drug indicated. Pharmacist will work with you and your healthcare provider to find solutions to any medication concerns. Patients are expected to bring all of the medications that they use to their appointment.  This included over the counter medications, prescription medications, puffers, creams, natural health products.
