Pharmacists can help patients manage their chronic conditions such as diabetes, high blood pressure, asthma, COPD and more. A pharmacist will meet with you for an intial assessment to gather your health information, identify any potential medication problems and discuss your goals with regards to your chronic condition and health as a whole. Together you and the pharmacist will create a plan which may include: changes to medications to allow you to meet treatment targets, ways to reduce/avoid adverse effects, training for home testing if applicable, and support/coaching to help you meet lifestyle goals such as diet/exercise/smoking cessation. Pharmacist will ensure you are up to date in other areas of care such as immunizations, screening and other tests including blood work if applicable. The pharmacist will provide you with a follow up plan to help support you in achieving your health goals. Follow up visits will be booked when needed.
Managing chronic disease follow up appointment: If you have had a previous assessment for your chronic condition, the pharmacist may request that you book a follow-up appointment.
