Speaker: Dr. Michael Vallis

Description: Collaborative Practice Demonstration Project Physicians and Pharmacists are invited to attend this session in person or via webinar. Dr. Vallis will build upon the concepts of Behavior Change presented in the online learning modules. He will use case studies from this project to discuss strategies that health care practitioners can use to support their patients in behaviors change. Specific examples will be used to demonstrate how to start and continue the conversation, and move patients along the behavior change continuum.
